Roadmaps fail in execution more often than they fail in design. The most common execution failure is not technical. It is the absence of shared commitment from the stakeholders who need to make decisions, provide resources, and tolerate disruption along the way. Building a roadmap in isolation and presenting it for approval is not the same as building a roadmap with the people who have to live with it. The difference shows up 90 days into execution when the first real prioritization conflict arises.
